Treatment by hand (manipulations) belongs to a group of mechanic-therapeutical procedures of physical medicine.Nowdays, manipulations, also called chiropractic are put into scientifically confirmed procedures which are applied worldwide in the treatment and specially with patients who suffer from certain spine changes. General characteristics of manipulative applications (passive and active mobilization, forceful passive movements) were explained in the article. Application of manipulations were presented in damages of lum- bal spine and morphological and biomechanical basis of this treatment was explained. Several most effective manipulations were presented that alleviate painful conditions within other measures of physical medicine (indepen- dent exercises, change of body posture, breathing exercises, mental and body relaxation). Key words:treatment by hand, manipulation, chiropractic, low - back pain

Scientifically tested treatment of diseases by manipulation has been widely approved as a medical procedure, while in our country it is still practiced in only few institutions. The authors have attempted to adjust extensive information on manipulation from the medical literature with experience gained at the Center for Pain Killing, Department of Neurosurgery, School of Medicine, University of Zagreb. Special attention has been paid to patients with inguinal spine disorders, manifested as low-back pain and lumboischialgia. The sample consisted of 60 patients (30 with low-back pain and 30 with lumboischialgia) treated at the Center with the procedure described in depth. The mean age of patients was 40 years, the youngest one having 29 and the oldest 46. In patients with low-back pain, the results were satisfactory in 18 (60%) patients, improvement was recorded in 7 (23.4%) and the treatment failed in 5 (16.64%) patients. In lumboischialgia, the treatment results were satisfactory in 17 (57%) patients, improvement was recorded in 8 (26.4%) and the treatment failed in 5 (16.6%) patients. Based upon extensive literature records and modest personal experience, the authors have come to a conclusion that manipulation treatment is a valuable supplementary procedure, with the aim of relieving discomfort and removing disorder in patients with inguinal spine disorders.
